Description
A. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. B. The Great Plains Area Indian Health Service (IHS) intends to award a firm-fixed price, non-personal service, single award, purchase order to provide Polysomnography Sleep and Home Study Services at the Quentin N Burdick Memorial Health Care Facility, Belcourt, North Dakota in response to RFQ 75H70626Q00101. C. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2025-04/2025-06-11. D. This RFQ is set aside 100% Small Businesses and the associated NAICS Code is 621498, which has a small business standard of $25.5 million. E. The quoted unit pricing must be “all inclusive” (to include but not be limited to travel, lodging, per diem, fringe benefits, federal, state and local taxes) plus all other costs pertinent to the performance of this contract. The purpose is to have the capability of providing polysomnography testing or sleep studies along with ancillary equipment that is required for Polysomnography lab testing and analysis that would be provided by the Contractor for Quentin N Burdick Memorial Health Care Facility. G. The period of performance will be twelve (12) months from date of award, with four (4) additional twelve-month option periods, for a total potential period of performance of five (5) years, if all options are exercised.
