Amendment 0001 The purpose of Amendment 0001 is to: a. Add document 'Attachment 4 - Questions and Answers Log' to address questions and answers received from industry. b. Provide vendor(s) information to obtain visitor's passes that are interested in a site visit prior to the solicitation closing date. Registration for Site Visit -Email both Damaris Castillo, Contract Specialist (damaris.i.castillo.civ@army.mil) and Amanda Ramirez, Contracting Officer (amanda.m.ramirez24.civ@army.mil) with the following information no later than Thursday, May 14th 2026 at 12:00PM MST: -The names of attendees (no more than four) and Company name - Email confirmation that the vendor has submitted and completed the Visitor's Pass. Visitors will need to stop at the Visitor Center prior to gaining access to the base. Visitors entering Yuma Proving Ground (YPG) in a POV must have: 1) a Government issued picture ID (Driver's License, CAC, etc.) 2) a current registration (or rental car agreement) 3) a current Proof of Insurance card YPG Visitor Center Location: Building 3537 off Imperial Dam Road Phone: (928) 328-6689 Website: https://home.army.mil/yuma/about/visitor-information -The site visit is scheduled for Monday, May 18, 2026, at 9:00 AM MST. The visit will include access to all elevator mechanical rooms and the general elevator areas accessible to personnel during normal use. Elevator shaft rooms and pit rooms will be restricted during this visit. c. Uploaded Attachment 5 - Solicitation Amendment W9124R26QA008001 to reflect the questions and answers log and the site visit information made in the solicitation. The Offeror shall sign the amendment to acknowledge the changes. Any current or future amendments, the Offeror shall sign them and include them when submitting proposals. d. No other changes to the solicitation have been made. REQUEST FOR QUOTE (RFQ) The United States Army Yuma Proving Ground (USAYPG), Yuma, Arizona is requesting a quote for is to provide comprehensive elevator maintenance and inspection services. The requirement includes monthly preventative maintenance, monthly inspections, routine cleaning, routine service orders, repairs, annual Arizona State certification performed by a state‑certified technician, and occasional emergency call‑outs for three (3) elevators located at the U.S. Army Yuma Proving Ground. This will result in a non-personal services contract. The Contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items necessary to perform services specified in Part 5 of the PWS. This is a combined synopsis/solicitation for commercial products or commercial services prepared in accordance with part 12. This announcement constitutes the only solicitation.
