This is a combined synopsis/solicitation for commercial services prepared in accordance with FAR Part 12. This announcement constitutes the only solicitation. Quotes are being requested and a separate written solicitation will not be issued. Solicitation No. 75H70726Q00060 is issued as a Request for Quotation (RFQ) for one (1) Registered Dietitian (RD/RDN) to provide Outpatient Medical Nutrition Therapy services for the Taos-Picuris Service Unit (TPSU) located in Taos, NM. This acquisition is set-aside for Indian Small Business Economic Enterprise (ISBEE) concerns. This solicitation incorporates provisions and clauses by reference. The full text of provisions and clauses may be accessed electronically at www.acquisition.gov. (ii) See attached RFQ and/or Fillable Table for Contract Line Items (CLINs). (iii) See attached RFQ for description of requirements. " Instructions to Quoters" begins on p. 14. (iv) Outpatient Medical Nutrition Therapy services to be provided for the following Albuquerque Area Indian Health Service (IHS) facility: Taos-Picuris Health Center 1090 Goat Springs Road Taos, NM 87571 (v) See attached RFQ for a list of solicitation provisions that apply to the acquisition.
