This is a Request for Quote to provide Ground Ambulance Services in accordance with the Performance Work Statement (PWS) contained herein. Refer to the Addendum to FAR 52.212-1, Instructions to Offerors, and VAAR 852.273-73 for response instructions. Please ensure all signatures are provided - on the original SF1449, all attachments, and any amendment SF30s. Note: Changes to the Schedule, Performance Work Statement, or Clauses are unacceptable as this is an open-market commercial Request for Quote and may result in a nonconforming quote.
