Statement Of Work CONTRACT TITLE: Elevator Inspections & Certification PLACE OF PERFORMANCES: VA San Diego Healthcare System 3350 La Jolla Village Dr. San Diego, CA 92161 PERIOD OF PERFORMANCE: One base year and four (4) option years. CONTRACT TYPE: Firm Fixed Price BACKGROUND: This contract is for semi-annual inspections and maintenance review, annual/CAT 1 inspections, and safety testing of Government owned Vertical Transportation Units (VTUs) at the VA San Diego Healthcare System, to include elevators, dumbwaiters, wheelchair lifts, and cart lifts. All VTUs additionally require a 5-year/CAT 5 elevator test. GENERAL REQUIREMENTS: 6.1. The Contractor shall furnish all equipment, expertise, incidentals, labor, materials, parts, supervision, supplies, tools, transportation, and qualified personnel required and necessary to conduct inspection services in accordance/compliance with manufacturer guidelines/recommendations, current code(s), directives, regulations, Joint Commission, and industry standards at VA San Deigo Healthcare System. 6.2. Contractor shall check in with Electric Shop Supervisor or appointed prior to performing any work. 6.3. Contractor and employees shall be issued temporary ID badges from the VA Police Department that must always be worn while on station. 6.4. All damages incurred during the performance of this scope of work will be repaired by the Contractor at no cost to the VA. 6.5. Contractor is responsible for providing and utilizing all OSHA regulated Personal Protective Equipment (PPE) while performing all inspection and testing requirements. 6.6. Contractor shall follow all VA regulations and provide the means to meet these regulations.
